Technology Responsible Use Form
Students must turn in a completed Technology Responsible Use Form (TRUF) in order to use any of the school technology devices and the school network. A copy of the form can be downloaded below.

Student Publication/Audio/Video Release Form (SPAV)
The Student Publication/Video/Audio Release form grants parental consent for a wide range of school activities and programs, including the participation in Distance Learning. A complete list of covered uses can be found on the SPAV form below. Parents must make sure to check either the "yes" or "no" box before submitting the SPAV form, otherwise the form will be marked as invalid.
